Through compassionate, trauma-informed, and culturally competent Home and Community-Based Services (HCBS), we equip families with the tools, support, and advocacy they need to nurture resilience, independence, and lasting well-being.
We collaborate with care teams, honor the dignity and individuality of every participant, and uphold the standards of New York State’s Children’s Waiver to ensure services are person-centered, need-driven, and rooted in meaningful outcomes.

Our providers are caring, qualified professionals who are trained to deliver high-quality, individualized services. Each team member meets rigorous state requirements, including background checks, and is committed to helping youth gain skills for greater independence and success at home and in the community.
Respite Services provide short-term, planned or crisis relief to families and caregivers of children and youth with developmental, medical, or behavioral health needs. Respite is delivered in-home or in an approved community setting and offers caregivers a temporary break while ensuring the child or youth receives supervision, support, and meaningful engagement in alignment with their individual needs and goals.
CFASS are short-term services that provide training, coaching, and education to parents, caregivers, and family supports. These services help reduce stress, build resilience, and improve the family’s ability to care for a child with medical, developmental, or mental health needs. CFASS also supports the child’s independence and helps families access needed resources to achieve identified goals.
Community Habilitation supports children and youth with developmental disabilities in gaining, maintaining, or enhancing skills needed for greater independence and participation in home and community life. Services are individualized and focus on building age-appropriate skills in areas such as daily living, communication, socialization, and health tasks, with the goal of promoting inclusion and reducing the need for institutional care.
Prevocational Services help youth ages 14 and older with developmental, medical, or mental health needs build transferable skills for future paid or volunteer work. Services focus on soft skills like communication, task completion, following directions, problem-solving, and workplace expectations, including safety. Activities are tailored to individual strengths and needs to support greater independence in employment settings.
Supported Employment helps youth ages 14 and older with developmental, medical, or mental health needs prepare for, obtain, and maintain paid employment in their communities. Services include ongoing supports such as job development, job coaching, benefits counseling, and career advancement planning. The goal is to promote long-term employment success based on the individual’s strengths, interests, and needs.
Day Habilitation provides structured, in-person support at certified sites for children and youth with developmental disabilities. Services focus on developing and maintaining skills in self-care, communication, socialization, and daily living to promote greater independence and community inclusion. Activities are designed to help participants build relationships, make informed choices, and actively engage in their communities.
HCBS stands for Home and Community-Based Services, a set of supports for children and youth with developmental, medical, or mental health needs. These services are designed to help children remain safely at home and involved in their communities, rather than being placed in institutional settings. HCBS also supports families and caregivers through education, skill-building, and short-term relief services.
Services are delivered in the child’s home or in community-based, non-institutional settings. Planning and intake meetings can take place in-person at our office, in the home, in a preferred community location, or virtually—whichever is most comfortable for the family.

Eligibility is determined by a care manager through a formal HCBS Eligibility Determination process.

Bronx, Kings, New York, Nassau, Orange, Queens, Richmond, Rockland, Suffolk, Sullivan, Ulster, and Westchester.
